These guidelines are not intended to direct the course of clinical care you provide to an individual Plan My Health member. Neither do these guidelines replace your independent professional clinical judgment or your professional duty to exercise your special knowledge and skill in the treatment of your patients. As set forth in your participating provider agreement, you remain responsible for the quality and type of health care services provided to Plan My Health members.
These guidelines were adopted from nationally known organizations such as the HEDIS (Health Plan Employer Data Information Set) initiatives and the different wellness and disease management programs we offer our members.
